The participation of any doctor, hospital or other health care provider is not guaranteed, even if a member is referred by his or her physician to the provider.
If you don't know which plan you are on, please consult with us before continuing. Searching in the improper plan type may result in finding a doctor that is NOT covered by your particular plan. You can call our Customer Service department at the telephone number shown on your Coventry member ID card. Representatives are available Monday through Thursday from 8:00 a.m. to 6:00 p.m. Central, and on Friday from 8:00 a.m. to 5:00 p.m. Central.
